Licensed Nursing Assistant – Hospice Community

Paraprofessional - Per Diem - Non-Benefitted

The LNA performs personal care and designated health-related services for Hospice patients.

Minimum of 1 year of LNA experience and NH L.N.A. license is required.

Is able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

Positions are available for both the Hospice House and community. Travel is required for all community positions.

 

RN- Hospice House

Professional - Full Time - Per Diem - Benefitted - Non-Benefitted

The Concord Regional Visiting Nurse Association is seeking a skilled RN to care for patients at our Hospice House with end of life care needs. The qualified candidate will be licensed by the State of New Hampshire as a RN with a MINIMUM OF 2-YEARS RECENT ACUTE CARE EXPERIENCE SKILLS IS REQUIRED. Previous Hospice and/or oncology experience preferred. Position includes weekend, holiday, on-call and off-shift rotations as needed by the program.

Is able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

Per Diem Shifts Available: 11 pm - 7 am & 3 pm - 11 pm

Full Time Shift Available: 11 pm - 7 am

 

Licensed Nursing Assistant - Hospice House

The care is provided to patients under the supervision of a registered nurse. MINIUMUM OF 1 YEAR OF LNA EXPERIENCE and NH L.N.A. LICENSE IS REQUIRED.

Shift Available: 11 pm - 7 am

 

RN - Hospice Community

Professional - Full Time - Per Diem - Benefitted - Non-Benefitted

The Concord Regional Visiting Nurse Association is seeking experienced RN's to provide care to Hospice patients in the community. The RN works with the multi-disciplinary team to provide end of life skilled nursing care according to the patient’s plan of care.

Qualifications: Licensed by the State of New Hampshire as an RN; a minimum of two years nursing experience with recent acute care experience skills is required. Previous Hospice and/or oncology experience preferred. Rotating weekend, holiday and on-call responsibilities required.

Speech Language Pathologist – Home Care

Professional - Part Time - Per Diem - Benefitted - Non-Benefitted

CRVNA is seeking an experienced Speech Language Pathologist will deliver skilled care to clients in their homes. Responsibilities include initial and ongoing evaluations, developing and implementing a plan of care and managing a caseload of patients.

Travel, weekend and holiday rotations are required for this position. Qualified candidates will have a degree from an accredited program and a current NH license. A minimum of 2 years’ experience is preferred.

 

RN - Home Care

Professional - Full Time - Benefitted

A Registered Nurse to deliver skilled nursing care to home health patients according to a plan of care established by the patient’s physician.

Responsibilities include implementing a plan of care and managing a caseload of patients. Travel, weekend and holiday rotation is required for this position.

The qualified candidate current NH nursing license; recent acute care experience skills; a minimum of one year nursing experience. Includes rotating weekends, holiday and backup on-call responsibilities.

Current openings in the Team 3 area west of Concord.

 

Personal Care Service Provider

Paraprofessional - Per Diem - Non-Benefitted

A Personal Care Service Provider provides assistance with eating, bathing, dressing, personal hygiene, activities of daily living, preparation of meals and housekeeping chores such as bed making, dusting and vacuuming.

Qualifications: A mature person who is at least 18 years old. High school diploma or GED required. Dependable Automobile with Insurance ($100,000/$300,000 liability) for community work is required.

Is able to understand and carry out directions and instructions, record messages and keep records. Is able to read, write and speak English. Is physically and emotionally capable of performing the job requirements.

No experience necessary; we will train. Work available in both Facility and Community settings.

Per Diem. Evening and Overnight hours available.

Homemakers - Personal Home Services

Paraprofessional - Per Diem - Non-Benefitted

Performs homemaking tasks according to the care plan and the client's wishes. Thorougly complete all tasks such as dusting, damp/dry mopping, vacuuming, cleaning kitchen appliances and cleaning bathroom fixtures.

CRVNA offers on-the-job training to qualified candidates. No previous healthcare experience is required. Participants must demonstrate competency in several areas at the end of the training. Training begins with orientation on Monday (8am - 4pm) then continues with a variety of classroom and/or hands on experiences based on the staff members’ availability.

What kind of work is currently available for a Homemaker?

CRVNA currently has Homemaking work available in the home setting on a Per Diem basis. While this type of work doesn’t provide a guaranteed schedule of hours or a fixed schedule, it does offer a flexible schedule based on the staff member’s availability and the agency’s needs.

If you enjoy working independently, Per Diem work in our patient’s homes is available in Concord and the surrounding communities. Travel in all kinds of weather is required for this work.
