Concord Regional VNA Home Care is available for people of all ages in your own home, independent living community, assisted living community, and group home.

 

Home Care is for People Who Are:

  • Recovering from surgery or an accident
  • Recovering from exacerbation of a chronic illness
  • Coping with physical impairments
  • Dealing with the complications of aging
  • Prescribed a new medication
  • Diagnosed with a new condition

Our experienced healthcare team partners with you and your physician in determining what you need and fosters independence by helping you reach your recovery goals. Our team listens to your concerns and answers your questions in a timely manner. Additionally, our use of advanced technology and equipment such as Telehealth allows us to maintain frequent contact with you and your physician and to quickly respond to your health needs.

What Do Home Care Services Include?

Our skilled nurses offer:

  • Comprehensive assessments
  • Case management and coordination
  • Education about how to improve your health, prevent exacerbation of your illness, and manage your symptoms
  • Administration of your medications and treatments
  • Education on falls prevention and home safety
  • Nutritional counseling
  • Wound care management and education
  • Specialized infusion therapy
  • Transitional care management

Our physical therapists and physical therapy assistants offer:

  • Comprehensive assessments and development of a home exercise program
  • Education and treatment to improve your functional status
  • Support in helping you restore your optimal strength and physical functions

Our speech therapists offer:

  • Comprehensive assessments
  • Support in helping you restore functional speech, voice, language, communication, and swallowing

Our occupational therapists offer:

  • Comprehensive assessments
  • Support in helping you better adapt to perform activities of daily living

Our social workers offer:

  • Information on community and financial resources, and long-term care planning
