Concord Regional VNA Hospice Care is available for terminally ill individuals in their own home, independent living facility, assisted living facility, nursing home, and at Concord Regional VNA Hospice House.
Our experienced healthcare team is sensitive to your needs and wishes and treats you and your family with the respect and dignity you deserve. We partner with you and your physician and other healthcare providers in developing a plan of care while concentrating on the physical, social, emotional, and spiritual aspects of someone's life and well-being. Our team provides comfort care, pain control, symptom management, and emotional and spiritual support combined with education and grief counseling.
Hospice care only briefly touches a life near its end, but the time is made more peaceful for you and your family. With the help of hospice, you and your family can focus on what's important - celebrating life together and living as fully as possible.
