Concord Regional VNA Telehealth is advanced home monitoring technology that plays a key role in improving care for people who have a chronic illness (i.e. heart failure, diabetes, pulmonary condition, and hypertension) and who are at high-risk of re-hospitalization. Telehealth monitoring is used to augment nursing visits and engage you in necessary self-care to help you manage and prevent an exacerbation of your chronic illness.

Our skilled nurse at Concord Regional VNA reviews the information (i.e. blood pressure, weight, and blood sugar) that you electronically submit via the small, lightweight monitoring device in your home. The nurse then identifies medical changes and works with your physician on needed medication and treatment interventions.

What Do Telehealth Services Include?

Our skilled nurse offers:

  • Expert clinical review of your submitted information
  • Continuous education on your specific disease process
  • Recommendations for self-management techniques
  • Periodic clinical reports for your physician on the information collected that enables them to make quicker adjustments to medications and/or treatment when necessary
  • Transitional care management
